Q: Why does most hepatic abscess rupture not cause intraperitoneal dissemination?


Answer: Due to localization in adjacent spaces

The phenomenon of hepatic abscess rupture is rare because high-sensitivity early diagnostic modalities are now readily available. It is now mostly seen in highly remote resource-limited settings. The two most common risk factors for rupture are:

1. abscess diameter >6 cm
2. Coexisting cirrhosis

Fortunately, widespread intraperitoneal dissemination and contamination are rare, as most ruptures localize to the perihepatic, subphrenic, or pleural space.
